Congenital bicuspid aortic valve
Most common cause of aortic stenosis in a 70 yo pt
Senile or degenerative aortic calcification
Classic but rare EKG finding in PE
S in lead 1 and Q plus inverted T in lead 3 (S1Q3T3)
Most likely cause of secondary HTN: HTN in arms, but low bp in lower extremities
Aortic coarctation
Most likely cause of secondary HTN: Proteinuria seen w/ renal disease, hypokalmia
Hyperaldosteronemia
Most likely cause of secondary HTN: Tachycardia, diarrhea, heat intolerance
Hyperthyroidism
Most likely cause of secondary HTN: Hyperkalemia
Renal failure; Renal artery stenosis
Most likely cause of secondary HTN: episodic sweating, tachycardia
Pheochromocytoma
Elderly F w/ h/o cholecystitis p/w 5 day history of vague recurrent ab pain
Gallstone ileus
Pt in ER p/w painful, irreducible inguinal mass - next step
OR; Incarcerated inguinal hernia requires surgical correction
4 mo child p/w non-bilious vomiting despite changing formula from milk based to soy based
Congenital pyloric stenosis
Pt for F/U w/ bp 150/85, record states bp was 145/95 in visit one mo ago - next step
Re-check bp in 2-4 wks; 3 sep bp’s on separate occassions required to dx HTN
Glomerular disorder: Most common nephrotic syndrome in children
Minimal change disease
Glomerular disorder: Granular pattern of immune complex deposition and hypercellular glomeruli
Post streptococcal (or post-infectious) glomerulonephritis
Glomerular disorder: Linear pattern immune deposition on BM
Goodpasture’s syndrome
