Crytstal Arthritis Flashcards
Uris acid is a product of
Purine metabolism
Hyperuricaemia
Males > 0.42 mmol/L
Females >0.36mmol/L
Risk factor of gout
Huperuricaemia Alcohol persistent consumption High BMI Old age Male
Clinical features
Severe pain/rapid onset
Always in singular joint first
Technique to diagnose
Arthrocentisis
Diagnosis of gout and pseudogout
Gout: negative biophringent crystal
Psuedogout: positive biophringent Crystal (ureic acid crystals)
Prevention strategies
Dietary restriction exogenous purines
Reduce drugs that reduce renal clearance
Stop taking drugs with endogenous purines
Acute setting strategy
NSAID (naproxen) 500mg 5 days
When would your acute treatment change
If patient is suffering with kidney disease we give presdinolone instead of NSAIDs (naproxen)
Drug to prevent reoccurrence
Xonthine oxidase inhibitors
Pseudogout other name
Calcium pryophosphate disease
What happens in pseudogout
Lose cartillage and develop calcium crystals
Risk factors
Age
Prior joint trauma
Haemochromotosis
Urges of pseudogout
Rapid onset Peaks in 24 hours Fever Erythema Resolution in 3-4 days
Pseudogout crystals
Small ramboid shape positive biophringent crystal
