overproduction causes of gout? (2)
malignancy
severe proliferative psoriasis

symptoms in gout?
hot, painful and swollen joint
gout presentation?
single acute hot, swollen painful joint
what are gouty tophi?
deposits of monosodium urate crystals that typically affect small joints (PIP)
RF for gout? (4)
male
obesity
alcohol
diuretics
what can be seen on gout x-ray?
dlytic lesions
punched out erosion
sclerotic borders
important point for hyperuricaemia?
do not treat if asymptomatic
medication for acute flare?
NSAIDs
Colchicine (runs)
Steroids
treatment for attacks?
1st attack not usually treated unless single polyarticular gout, urate calculi, renal insufficiency
2nd attack to be treated within 1 year
wait until attack has settled before lowering UA
medications for maintenance? (3)
allopurinol - xanthine oxidase inhibitor
febuxostat
canakinumab
what is the deposition in pseudo-gout?
calcium pyrophosphate dehydrate crystals
who does pseudo-gout happen to?
the elderly, particularly females with erratic flares
treatment is like gout but if very severe what can you do?
arthrocentesis
