what is croup?
laryngotracheobronchitis, a subglottic viral infection that casuses increased a/w swelling that can advanced to obstruction
which age groups tend to be affect most by croup?
> /=6months to 3 years
s/s of croup
hx of or current URTI, slow onset Low fever, runny nose, narking cough at night and symptoms worsen at night, tachy cardia, tachypnea
severe croup can have
alt loa, accessory muscle use, inspiratory and expiratory stridor, cyanosis and all the other s/s.
epiglottis
supraglottic bacterial infection twith a quick onset of high fever in ages 2-7 yrs old.
s/s of epiglotitis
drooling, sudden high fever, cyanosis, sniff position, can progress to complete a/w blockage ( don not inspect the a/w)
managment of croup
blow-by o2, NEB epi, cold/humidfy,
Croup conditions: directive
Epi= >/=6mth-<8yrs, hr<200bpm, stridor at rest
Dex:>/=6mth-<8yrs, Unaltered, mild-moderate, to severe croup
Treatment for croup Medical directive?
Epi:
<10kg, 2.5mg NEB 1
>/= 10kg, 5.0mg NEB, 1
dex:
-Po, 0.5mg/kg, 8mg (max), 1 dose
Croup contraindications
Epi: a/s
dex: a/s and pt on po or parenteral steroids in the last 48hrs
O2 setting for NEB?
6-8L
you should not examine the pt throat if you suspected croup or epiglottis (t/f)
