what is the ped chain of survival?
prevention, activate emergency response, high quality CPR, advanced resuscitation, post-cardiac arrest care, recovery
steps of CPR (Peds BLS Algorithm)
1) verify scene safety
2) check for responsiveness and shout for help
3) check for breathing and pulse (check pulse for 5-10s); monitor pt if yes to both
4a) no pulse or HR <60/min w/ poor perfusion? –> chest compressions
4b) no breathing, yes pulse or HR >60/min? –> rescue breaths: 1 breath every 2-3 seconds aka 20-30 breaths/min; check pulse q 2 min
5) if still alone and no pulse after 2 min of CPR, get AED
6) if AED detects shockable rhythm, give 1 shock; check AED q 2 min, otherwise cont CPR
how to do rescue breaths for peds?
1 breath every 2-3 seconds aka 20-30 breaths/min; check pulse q 2 min
how to check pulse on infant vs child
brachial pulse: 2-3 fingers in inner upper arm midway elbow and shoulder vs carotid pulse or femoral pulse: 2-3 fingers on inner thigh b/w hip bone and pubic bone (just below crease where leg meets torso)
what’s the compression to ventilation ratio for peds? what’s the compression rate for peds? what’s the compression depth for peds?
30:2 (15:2 if 2 rescuers). 100-120/min. for infant: 1.5in; for child: 2 in
when to switch from rescue breathing to CPR for peds?
signs of poor perfusion, no pulse felt, HR < 60/min w/ poor perfusion
what is respiratory arrest and what are the signs?
when nml breathing stops; victim = unresponsive, not breathing or only gasping and still has a pulse
how to give mouth to mouth breathing for children?
same as adults:
1) head tilt-chin lift
2) pinch nose closed w/ thumb and index finger, rest other fingers on victim’s forehead
3) take nml breath and seal your lips on victim’s
4) deliver 1 breath/second and watch chest rise. if chest doesn’t rise, rpt head tilt-chin lift an give second breath. if chest still doesn’t rise (after 2 attempts), do chest compressions
how to give mouth to mouth breathing for infants? how to give mouth to mouth and nose breathing for infants? what’s the preferred technique for infants?
same with children and adults. same as mouth to mouth except you give breaths to the nose. mouth to mouth and noses = preferred for infants
what is gastric inflation?
when you give breaths too quickly or too much that the air goes to the stomach instead of the lungs; occurs in mouth to mouth, mouth to mask, or bag-mask ventilation
where to place hands for chest compressions in infants?
2 fingers on center of chest JUST BELOW NIPPLE LINE; or, 2 thumbs encircling hands technique on center of chest JUST BELOW NIPPLE LINE
how long to chk pulse?
5-10 seconds
Examples of advanced airways? How does CPR change when they’re present?
Laryngeal mask, endotracheal tube, supraglottic airway. Everything is the same except don’t pause for rescue breaths during chest compressions; if only giving rescue breaths, then 1 breath q 6s for adults and 1 breath q 2-3s for pets
