Spastic hemiplegia :
Hyperpronated flexed arm , hand fisted .
Spastic Quadriplegia:
Scissoring of the legs ,
pronated forearms ,
fisted hands.
Spastic CP :
Increased tone , increase reflexes, clonus , clasp knife.
Intrathecal Baclofen
Global spasticity Potentiates central inhibitory neurons by its GABAb Agonist action
complicatin :
Severe spasms and pain, myoglobulinemia Progressive increased scoliosis
Selective dorsal rhizotomy
Difficult to control (often global) spasticity a proportion of the nerve roots in the spinal cord are selectively cut
Complications:
tone/dystonia Spinal deformity weakness and crouch
Botulinum toxins injections
Focal Hypertonia Reduce Spasticity and the pain Reversible local muscle weakness
Complications
Urinary incontinence Muscle weakness and stumbling Weakening of nearby muscle Aspiration
