A
1) establish caring relationships
2) assessing patients knowledge, attitude, physical and mental capability
3) providing visual aids in addition to oral information
4) verifying patients’ understanding

A
- new med, ask if they know the name - what is it for
- confirm address, identification
- allergies - confirm why it is okay to take or not
- if on any other meds, can arrange a separate
- counselling on those specific meds
- ask if doctor told them HOW to take the medication (MDP team links)
- what do to if there if side-effects, stop med?
- explain how; forget a dose, storing? pregnant? when to take? diet/excise.
- non-clinical advice
- what did doctor say to expect from this medication - benefits? e.g. lower blood pressure
- open questions
- get patient to explain how to take the medication
