  • new med, ask if they know the name - what is it for
  • confirm address, identification
  • allergies - confirm why it is okay to take or not
  • if on any other meds, can arrange a separate
  • counselling on those specific meds
  • ask if doctor told them HOW to take the medication (MDP team links)
  • what do to if there if side-effects, stop med?
  • explain how; forget a dose, storing? pregnant? when to take? diet/excise.
  • non-clinical advice
  • what did doctor say to expect from this medication - benefits? e.g. lower blood pressure
  • open questions
  • get patient to explain how to take the medication
