What are contraindications to IUD
Known STIs
Pregnancy
Active PID
Undiagnosed vaginal bleeding
Genital tract malignancy
Uterine anomalies
Copper IUD: Wilson’s disease
LNG IUD: hormone sensitive breast cancer
What are side effects of LNG IUD
Headache, breast tenderness, nausea, depression
What is the MOA of LNG IUD
Inhibits sperm migration and viability
Endometrial suppression
Altered cervical mucus
What is the MOA of Copper IUD
Inhibits sperm migration and viability
What are noncontraceptive benefits of OCPs
Regular menses
Less blood loss
Decreased iron deficiency anemia
Decreased PID
Decreased dysmenorrhea
Decreased incidence of endometrial and ovarian cancer
Decreased functional cysts and benign breast disease
Treatment of endometriosis
Decreases acne and hirsutism
OCPs and HTN
Not recommended for BP >140/90, even if well controlled
Contraindicated if >160/100
OCPs and DM
Acceptable unless >20yrs duration of DM or if microvascular disease present
OCPs and Migraine
Increased incidence of stroke in migraines with aura
Progestin only or IUD should be used
OCPs and breast cancer
Low dose OCPs does not increase risk for BRCA1 and 2 pts for breast cancer and DOES decrease ovarian cancer risk
OCPs and medications
Anticonvulsants: can decrease OCP efficacy or may reduce drug level
Antibiotics: OCPs and Rifampin contraindicated
Antiretrovirals: OCPs and fosamprenavir contraindicated
When do you stop OCPs preoperatively
One month prior to surgery if prolonged immobilization is expected post op
OCPs and Obesity
Slight increased risk of VTE
OCPs and SLE
if mild and without antiphospholipid antibodies, can use OCPs
What are absolute contraindications to OCPs
Breast cancer
Any estrogen-sensitive tumor
Pregnancy
Unexplained vaginal bleeding
Known inherited thrombophilia
Thrombosis
Smoker 35years or older with 15 or more cig/day
Liver disease
Migraine with aura
Known ischemic heart disease, hx of CVA
