What are the components for Consent to Assess a client?
Confidentiality of the Health history Why it is necessary Brief description of the assessment What the client will wear & positioning Areas to be touched or observed Assessment may make symptoms worse momentarily Describe Symptoms during assessment Does client have any questions Does the client consent to assess
When asking the client if they consent to Assessment does it have to be verbal or Written?
It must be Verbal not written.
What do Assessment Results give you?
History of clients health present Symptoms And Chief Complaint overall tissue health Functional Ability Structures to be involved in treatment Helps make a clinical impression of the clients condition Helps rule out other conditions CI's if the Client needs to be referred out of the clinic
What is the SOAP chart?
It is a universal form of documentation used for effective communication with clients.
It states:
Subjective, Objective, Assessment & Plan.
What is the Subjective portion of SOAP?
This is the info the client is telling you based on how they feel.
What is the Objective Portion of SOAP?
This is the info gather through testing and observation.
What is the Assessment portion of SOAP?
This is the Clinical impression based on palpation, ROM, postural Assessment, orthopaedic testing, and Gait analysis.
What is the Plan portion of SOAP?
This is the treatment used on the client, Frequency of treatments, Self care, CI’s, precautions and Referrals.
What type of pain would be from superficial tissue damage?
This would be Cutaneous pain.
The pain is sharp, bright and burning.
What type of pain is associated with nerve root compression?
This is called Radicular pain & can present as Sharp, Shooting and lightning like.
