Antibodies present in autoimmune
Anti Ro, Anti La, anti nuclear
anti double stranded RNA
anti centromere, ant SCL 70
Antineutrophil cytoplasmic
Systemic lupus erythemitosis
organ changes and antibodies
anti nuclear, anti double stranded DNA and anti Ro
affects joints, skin, kidney, blood, muscles
Common women of child bearing age and CVS risk
butterfly zygomatic rush, raynauds pehnomenon, heart problems, arthritis
Dental aspects of systemic lupus
chronic aneamia - GA risk and oral ulcerations
bleeding tendency due to thrombocytopenia
renal disease - impaired drug metabolism
lichnoid reactions
steroids and immunosupression
oral pigmentation from hydroxychloroquine
Antiphospolipid antibody syndrome
marker in blood called lupus anticogulant
recurrent thrombosis - DVT with pulmonary embolism - on warfarin
Sjorens syndrome
inflammatory disease assoicated with circulating antibodies
not caused by them (anti Ro, LA and anti nuclear)
involves salivary glands
Signs - dry eyes and mouth
Dental aspects of sjorens syndrome
Oral infection, caries, denture retention
sialosis (swelling of salivary gland)
salivary lymphoma
Sysyemic sclerosis
excessive collagen deposition, connective tissue fibrosis and loss of elastic tissue
Features - raynauds, osephagus might not stretch for food and gut not allow easy peristailis
Dental aspects of systemic sclerosis
limited mouth opening, poor oral access, limtied tongue movement
Reflux so erosion likely
widening of PDL but no tooth movement
Vasculitis
inflammation of blood vessels
Giant cell artierties
headache and facial pain present
involves other carotid branches - chewing claudication occlusion of central retinal artery
Polymyalgia Rhemiumatica
elederly - large blood vessels affected
pain and morning stiffness
responds to steroids
malliase, weight loss, fatigue
Kawaski Disease
occurs in children fever, lymphadenopathy, cracked tongue strawberry tongue, erythemetous mucosa, peeling rash on hands and feet coronary vessel anerysums - may need antibiotic prophylaxis
Wegner’s Granulomatosis
Inflammatory - can lead to distruction of hard and soft tissue of face and oral cavity
Spongy red tissue
renal and respratory affected
Fibromylgia
symptoms but no evidence of acute disease
joint and muscle pain, functional disorders
