Right sided failure physical exam findings
Peripheral edema, ascites, hepatomegaly, JVD
Left sided failure findings
Tachypnea, dyspnea, rales, crackles, pulmonary edema, cyanosis (late), PND, orthopnea

EF
Systolic dysfxn
Failure of the pump function of the heart.
Characterized by decreased EF
Generally caused by dysfxn of cardiac muscle
Can be caused by cocaine, ethanol
Scar formation after myocardial infarction
How does systolic dysfxn lead to pulmonary edema?
Ventricle inadequately emptied, EDP and volume increased.
Transmitted to atrium and pulmonary vasculature.
Increased pressure in lungs, and pulmonary edema (and peripheral edema on the R side)
Diastolic Dysfxn
Usually results from a non-compliant ventricular wall.
Leads to inadequate ventricular filling, decreased stroke volume.
Can lead to an end result that looks like systolic dysfxn.
How does diastolic dysfxn present physiologically?
The patient may be exquisitely sensitive to exercise.
A fib leads to flash pulmonary edema

Class I Functional Classification
No limitations are experienced in any activities.
Class II Functional Classification
Slight. Mild limitation of activity
Patient is comfortable at rest and with mild exertion.
Class III Functional Classification
Marked limitation with any activity
Pt is comfortable only at rest
Class IV Functional Classification
Any physical activity brings on discomfort and symptoms occur at rest.
