Idiopathic Dilated Cardiomyopathy (IDC)
A disease of unknown etiology that principally affects the myocardium
LV dilation, systolic dysfxn
Dilated cardiomyopathy
Ventricles stretch and become flabby Cause often unknown Drug toxicity Positive feedback cycle Impaired systolic fxn
IDC presentation
Heart failure Anginal CP Emboli Syncope Sudden cardiac death Arrhythmias
Mgmt of DCM
Limit activity Salt/fluid restriction ACE, diuretics Digoxin Anticoagulants Anti-arrhythmics
Peripartum Cardiomyopathy
Ventricular dysfxn form one month to delivery to 5 months postpartum.
Rare
Hypertrophic cardiomyopathy
Inappropriate, asymmetrical myocardial hypertrophy.
Septum is usual site for hypertrophy
Familial and sporadic forms
Diastolic disorder
Which heart sound will be heard with HCM?
Palpable S4
HCM Tx
Vasodilators, Beta blockers, diuretics
SUrgery
ICD, pacemaker
Alcohol septal ablation
Restrictive Cardiomyopathy
Least common if the cadiomyopathies
Diastolic dysfxn
Depressed contractility
Atria are enlarged because of high pressure
Restrictive symptoms
Right sided symptoms
Thromboembolic episodes
JVD, S3, S4
Amyloid Heart Disease
Thickened, firm rubbery ventricular muscle Major diastolic dysfxn Possible systolic dysfxn Low voltage on EKG Abnormal Q waves Widespread thickened walls
Myocarditis
Heart becomes inflamed Usually viral in the US Cardiac sx following FLS Angina like pain S3 Tx with bed rest
