COMPOUNDING LABELING
The dispensed container for any compounded preparation shall bear a permanently affixed label with at least the following information:
Name of the prescriber (in a retail pharmacy only, or if the preparation is intended for outpatient use)
Patient name
Directions for use
Name of all active ingredients
Name, address, and telephone number of the pharmacy
“Use by” date
Ancillary and cautionary instructions as needed
As pertinent, a warning consistent with applicable Federal and State law, that antineoplastic agents and other hazardous substances products are biohazardous
As pertinent, the requirements for proper storage
Additional requirements for sterile compounded preparations include:
Prescription number (in a retail pharmacy)
Date and the time prepared
Strength or quantity of all active ingredients, and the name and volume of the diluent, vehicle, and base solution(s), if applicable
“Use by” time (in addition to the date required), e.g. 12:00
Name or identifier of the person who prepared the product (if person who prepared the product does not witness administration)
