Allowing healing enables
Some remodelling of bone to occur to provide best surface for denture fit
Ideal pathway of conversion from dentate to edentulous
Inadequate dentition
Transitional Partial denture
Transitional complete denture
Permanent complete denture
If OHI is good, partial and complete overdenture can be considered
Indications for immediate dentures
Remaining teeth are health risk e.g pre-radiotherapy
When fewer visits are preferred (cost/time)
Where teeth are so misaligned or overerupted that transitional dentures are impossible to provide
When status/prognosis for remaining teeth is hopeless
Contraindications for immediate dentures
Elderly
Large bony undercuts
Difficult extractions
Immediately post extraction
No room for flange - lack of security
Compromise needs to be made in interim
Significant factors in assessment
Current RPD - use as transitional - but assess first
History of nausea - test with PD
Undercuts
Prior to tx
Warn patient that immediate denture will not be permanent
Will need to be replaced by permanent denture when resorption process has settled
Adequate emergency follow up
Immediate complete process
Placed immediately prior to extraction (LA)
Oedematous tissue inflammation helps fitting
Kept on for 24hrs
Return next day to remove and clean extraction site use tissue conditioner if hard to place back on
1st day 1st week 1st month 6/12 12/12
Adjust fitting surface and occlusion Correct pain and advise fixative use Further corrections Permanent reline Replace with permanent dentures
