Classic Clinical Features
Pain and paresthesias ->
Paralysis and Pulselessness
Clinical Confounders of Hand Compartment Syndrome
May not be a/w paresthesias
Extremely subtle motor deficits
Difficulty assessing passive stretch
Clinical Features of Hand Compartment
Pain is deep, constant, poorly localized, out of proportion
Extreme swelling
Intrinsic minus position at rest (MCP joing extended with PIP joint slightly flexed)
Pain with passive stretch of the involved compartment
Passive stretch maneuver of Interosseus
MCP joint extended and PIP joint fully flexed
Slight radial and ulnar deviation
Passive stretch maneuver of Thenar, Hypothenar
Extension of MCP joint
