Common Elbow & Wrist Problems Flashcards
What does FOOSH stand for?
Fall onto out-stretched hand
Describe an elbow dislocation
Children, young adults, pain/deformity/loss of function, usually posterior, can have fractures
Outline a supracondylar fracture distal humerus
fracture of the distal humerus just above the epicondyles, young children, pain, swelling, bruising, no function, neurovascular compromise, un/displaced
Describe a radial head/neck fracture
FOOSH, Commonest elbow fracture, Lateral pain/modest swelling/loss of range, Fat pad’ sign,
Outline subluxation of the radial head (pulled elbow)
Subluxation from annular ligament, Longitudinal traction along extended arm, 2-5 yrs, Reduced move/lateral pain/‘not using it’
What is closed reduction?
procedure for putting the pieces of a broken bone back into the right position without surgery
Outline elbow osteoarthritis
Degenerative disease of articular cartilage, M>F 4:1 Manual workers, Symptoms relatively rare, Stiffness, loss of extension, Surgical Debridement
What is elbow rheumatoid arthritis?
Systemic Autoimmune disease, Commonest inflam arthropathy, Synovial pannus -> joint destruction, Symmetric Polyarthropathy with morning stiffness
Outline lateral elbow tendinopathy
Tennis elbow: Degenerative tendinopathy - Common Extensor Origin, Commonest cause of elbow pain, Beware - Radiating pain from neck or shoulder
Describe medial elbow tendinopathy
Golfer’s Elbow’, degenerative tendinopathy - Common Flexor Origin, 10x less common than LET,
What are rheumatoid nodules?
Extra-articular manifestation of arthritis, treatment: disease-modifying antirheumatic drugs (DMARDs)
Describe olecranon bursitis
Inflam of bursa due to trauma/friction
Outline gouty tophi
Disorder of nucleic acid metabolism = urate crystal deposition, treated with meds
What is a colles fracture?
FOOSH, dinner fork, Dorsal displacement and angulation, shortening
What is a smiths fracture?
Palmar displacement and angulation, shortening
