  1. Amiodarone inhibits CYP2C9 (Major warfarin substrate) = DECREASED warfarin metabolism, INCREASED INR, bleeding risk INCREASED
  2. Start warfarin at decreased dose, or empirically decrease dose by 30-50%, monitor INR, adjust as needed.
