A
  • File Form 7A with the clerk, with a copy of the claim for each defendant, unless claim is filed electronically
  • clerk will then issue the claim by dating, signing and sealing it, and assigning a court file number.
  • A claim shall be served within 6 months after the date it was issued, but the court may extend this time

  • full names of the parties to the proceeding, the capacity in which they sue or are being sued
  • the nature of the claim w reasonable certainty and detail, including date, place, and nature of the occurences in which the claim is based
  • the amount of the claim and relief requested
  • the name, address, telephone number and LSO registration number if any of the rep, OR if self represented, the plaintiff’s address, phone no, email address
  • address where the plaintiff believes the defendant may be served
  • any documents on which the plaintiff’s claim is based
