Primary hemostasis
platelet adhesion (platelets stick to damaged endothelium- exposed subendothelial collagen with vwf, platelets bind to vwf) platelet aggregation- clump together after being activated and getting sticky
secondary hemostasis
extrinsic pathway- VII (tissue factor, normally sequestered, expressed on fibroblasts, activated endothelium and platelets)
intrinsic- hmwk, pk, XII, XI, IX, VIII
both lead to X, V, II, I (common pw)
XIII- fibrin stabilization
screening for primary hemostasis
platelet count- if low cannot form platelet aggregation
bleeding time
vwd can lead to prolonged bleeding time
fibrinolysis screening
d-dimer/fdp
test for DIC
pt
add tissue factor and phospholipid 2- activates extrinsic system
tests VII, and common pw proteins- X, V, II, I
high in liver disease, coumadin/ warfarin therapy
ptt
deficiency in intrinsic and common pathway
hemophilia A (VIII) and hemophilia B (IX)
