What is CLL?
Malignant proliferation of lymphocytes in the blood and bone marrow
Most important chronic lymphoproliferative disorder?
Chronic Lymphocytic Leukemia
Who does CLL occur in?
ONLY adults ( > 40 yo)
Things to know about CLL?
Small mature lymphocytes
same thing as small lymphocytic leukemia
B cell, CD5+
CD5+
CLL, considered a T cell marker but neoplastic B cells express
Why does infection occur in CLL?
hypogammaglobulinemia
What will you see in CLL smear?
Mature lymphocytes, condensed chromatin (smudgy)
MONOMORPHIC. They look like they are copied and pasted.
Phenotype of CLL cell
CD20 positive
CD5 positive
TdT negative
Treatment of CLL
conservative, reduce organomegaly and manage infection
Death from CLL occurs from….
infections
Things to know about Hairy Cell Leukemia
Hairy Cells
Splenomegaly without lymphadenopathy
Pancytopenia
TRAP positive
Clinical Findings of HCL
Male : Female = 5 :1
Older ( >40)
Pancytopenia and monocytopenia
Big spleen, no lymphadenopathy (spleen down to pelvis)
What will you see in a HCL smear?
Hairy lymphocytes
Fibrosis around cells
What does TRAP+ indicate?
Hairy Cell Leukemia
Immunophenotype of HCL?
Positive for B cell antigens
Negative for CD5
TRAP positive
