What is the Swiss cheese model?
A model of accident causation that illustrates that, although many layers of defence lie between hazards and accidents, there are flaws in each layer that, if aligned, can allow the accident to occur
What is second victim syndrome?
The suffering the health care provider experiences as a result of the psychological trauma the event causes
What are the steps of the second victim syndrome?
