Chronic pain can be classified into what 3 categories?
nociceptive pain
neuropathic pain
centralized pain
what is nociceptive pain?
pain caused by a stimuli that threated or result in bodily tissue or damage
what is neuropathic pain
pain resulting from maladaptive response to dame or pathology of the somatosensory nervous system
can occur in absence of active stimuli or as an exaggerated response to minor or moderate stimuli.
what is centralized pain?
reduced ability of the CNS to diminish responses to peripheral stimuli.
what is a key factor to chronic pain and it’s effects?
affects all aspects of life, family, and employment. common complaints are low back pain, joint pain, severe headache, neck pain and facial pain
besides biomedical causes, what are other causes of pain?
psychologic, social, sleep. need to do a full evaluation.
what is defined as “chronic pain”
painful condition that extends beyond the usually expected 6-12 week time course for healing.
How is nociceptive pain characterized
traditionally recognized as pain that is caused by noxious stimuli
how is neuropathic pain described?
tingling, pins, and needles, burning, shooting, like electric shoks.
some patient evaluation of patients with chronic pain syndrome
past medical and surgical hx review of systems social and family hx psychiatric history pain assessment: history of pain, location characteristics, severity, and impact of life.
initial management of of chronic pain is usually what kind of therapies first:
non pharmacologic.
e.g. home exercise programs, physical therapy.
what are some psychological approaches for patients with mood, sleep, quality of life issues?
Cognitive Behavior Therapy (CBT)
mindfulness therapy
Other random alternative therapies for chronic pain stuff…..
acupuncture (CHY-NA)
spinal manipulation with osteopathic manipulation therapy/chiropractic care
transcutaneous nerve stimulation.
meds that inhibit pain transmission
Tylenol, nsaid, capsaicin
