CLASS NOTES Thyroid Flashcards
Tx of hypothyroidism
Levothyroxine, Liothyronine, Liotrix
Mechanism of Action: Levothyroxine
Synthetic T4
Levothyroxine vs Liothyronin
Longer half life, slower peak duration, eliminated more slowly
SEs of Levothyroxine
Infants require more, angina pectoris requires lower initial dose
Mechanism of action: Liothyronine
Synthetic T3; used in combo w/Levothyroxine; MORE POTENT. Less desirable for chronic use; tx for myxedema coma
SEs of thyroid hormones
Hyperthyroidism symptoms
Thioamides (3)
PTU, Methimazole, Carbimazole
Mechanism of Action: Thioamides
Inhibits Thyroid peroxidase: inhibits organification of iodide and coupling of MIT/DIT
Safer in pregnancy: PTU or Methimazole?
PTU
Safer in agranulocytosis: PTU or Methimazole
Methimazole
SE’s of Thioamides
Agranulocytsis, Hepatitis, Lupus-like syndrome
Longer half life: PTU or Methimazole
Methimazole
Faster excretion: PTU or Methimazole
PTU
Less transplacental passage: PTU or Methimazole
PTU
Lower concentration in breast milk: PTU or Methimazole
PTU
