chronic dz of SI and LI Flashcards
what is inflammatory bowl dz (IBD)
broad term for condition with chronic or recurring immune response and inflammation of the GI; can be SI or LI or a mixture; 2 most common are ulcerative colitis and crohn’s disease
where is the most common location for Crohn’s disease
iliocolic junction
is irritable bowl syndrome IBD?
NO ! there is no inflammation present, it is a functional dz
what is the most common layer to get infiltrated during IBD?
lamina propria
IBD pathogenesis
- clinical signs attribute to the mucosal cell infiltrates and inflammatory mediators
- releases compliment, prostanoids, leukotrienes, pro-inflammatory cytokines, leukocyte proteases, nitric oxide, O2 derived free radicals
IBD clinical signs
-extremely varied, species differentiation, location and severity
IBD species predilections
- basenjis
- lendehunds
- soft coated wheaten terriers
- irush setters
IBD in cats
- vomiting is the most frequent sign, unrelated to eating
- lethargy, change in appetite (increase OR decrease), loss of weight and condition
- hairball accumulation
IBD in dogs
- diarrhea is the most frequent sign, small or large bowl, usually chronic
- vomiting may be present
- borborygmus and flatus
- halitosis
- abdominal pain
- pica, polyphagia
PE possibilities for IBD cat and dog
-normal
-weight loss
-cachexia
-thick intestine
-mesenteric LN large
CAT ONLY : no edema
DOG ONLY : edema/ ascites is possible
what is the key to diagnose IBD and lymphangiectasia
biopsy (but consider therapeutic trials first)
treatment for IBD
- deworm
- diet and antibiotic/ probiotic trials
- steroids
what are some antibiotics that we use for IBD
tylosin
amoxicillin
metronidazole / enrofloxacin
what are some steroids we use for IBD
pred
dex
budesonide
alternative meds for severe or non-responsive IBD
- azathioprine
- cyclosporine (for PLE)
- chlorambucil
is Histiocytic ulcerative colitis IBD
yes, just hard to find the inflammation
-due to E.coli
histiocytic ulcerative colitis clinical signs
hematochezia
tenesmus
what breeds mostly get histiocytic ulcerative colitis
boxers, frenchies, boston terriers
how do we treat histoicytic ulcerative colitis
antibiotics and NOT STEROIDS
-enrofloxacin, +/- amoxicillin/metro
diseases that can cause PLE
inflammatory
infectious
neoplasia
lymphangiectasia (primary or secondary)
what is the most common breed for lymphangiectasia
yorkies
