What is our duty as doctors in NAI?
Have a duty to act upon concerns we have about the safety/welfare of a child/young person
What happens after a referral has been made to police/social work?
Initial assessment to understand the risks to the child
Involves health visitor, social work, police, child protection case conference, child protection order
Child may be left at home or accommodate. may need compulsory supervision
What are the different types of abuse?
Physical Sexual Emotional Fabricated/induced illness Neglect
What should ring alarm bells in your presentation of a child that NAI may be going on?
Late presentation
Hx keeps changing/inconsistent with findings
What kind of bruising is likely to be NAI?
Linear/patterned
Bruising in a baby that is not mobile yet
What is the difference between a burn and a scald?
Burn - dry heat
Scold - hot liquid
How do you differentiate between impetigo and a cigarette burn?
Both circular & burns often infected
Look for distribution, healing and burnt hair
What things in fractures would you think NAI?
Baby not mobile yet
Rib fractures as ribs are very complaint so high force req. to break them
Spinal fractures may be resulting from twisting motion
What is the commonest cause of death in physical abuse?
Abusive head trauma
What do you see in most abusive head trauma?
Retinal haemorrhages
Neck and cervical spine injury
What are the types of neglect?
Emotional Abandonment Medical FFT/nutritional Educational Physical Failure to provide supervision
What things may indicate sexual abuse?
Disclosure
Sexualised behaviour
STI
Pregnancy
