Chapter 7 : Tracheotomy Flashcards
Four indications in tracheostomy
SPAM
- Pulmonary toilet
- Prolonged Mechanical ventilation
- Surgical access
- Airway obstruction
Time frame that differentiates early vs late complications
Early < 7 days
Late ≥ 7 days
Surgical landmarks in tracheostomy
Hyoid, thyroid, cricoid
Difference between tracheotomy vs tracheostomy
Tracheotomy - creation of a stoma in the trachea.
Tracheostomy - creation of a permanent stoma by suturing to the skin.
Indications for a vertical incision vs horizontal incision
Vertical incision : emergencies or indistinct landmarks
Horizontal incision : elective, distinct landmarks
Normal capillary perfusion pressure of the trachea
25 - 30 mmHg
When are cuffed tubes needed?
M-A-P
1) Patients on Mechanical ventilation
2) Aspiration risk
3) Needing Pulmonary toilet
Risk factors for tracheocutaneous fistula
B-RaT
- Bjork flap
- Radiation therapy
Requirements for decannulation
- Adequate mentation to protect airway
- No need for further intubation
- Able to tolerate capping
- Able to uncap if with difficulty breathing
Usual site of bleeding in tracheostomy
Anterior jugular vein injury or bleeding edge of the thyroid
