levothyroxine for thyroid hormone replacement

A

Synthetic T4

Highly individualized doses

Increase dose at 4-6 weeks if have CVD
Serum TSH levels regularly monitored
Admin medication at same time every day

-High TSH=need higher dose

AE:

  • hyperthyroidism
  • Black box*: Use of thyroid hormone in treatment of obesity or weight loss is contraindicated.

methimazole (Tapazole) for hyperthyroidism

A

Interferes w/ synthesis of T3 and T4
Effects:
-Normal thyroid function: weight gain, lower anxiety, less insomnia, lower HR

-*Notify provider immediately if pregnancy expected

AE:
-Hepatotoxic

Don’t use with warfarin

Hydrocortisone pharmacotherapy

A

Corticosteroid

mimics cortisol- can treat:

  • Inflammation
  • allergic reactions
  • Can be injected into inflamed joints
  • Use correct route
  • Admin same time every day

AE:
-Cushing’s symptomsFluid retention
CNS effects

Don’t use if have an infection

Desmopressin (DDVAP) pharmacotherapy

A

Synthetic ADH->increase reabsorption of water

Used to control acute DI

Prod. clotting factor VIII->bleeding management for hemophilia A and von willebrand’s.

Can be used for fixing betwetting/nighttime polyuria (less urine production)

-After IV restrict fluid to prevent water intoxication

AE:

  • Water intoxication
  • Black box: Can cause hyponatremia

Don’t use if DI caused by kidney disease.
