Reason for seeking care (Not a diagnostic statement, brief statement in patients own words providing reason for visit). Present health history or history of illness. Past health. Family history.

A symptom is a manifestation of disease apparent to the patient himself, while a sign is a manifestation of disease that the physician perceives
