Optimal Nutrition Status
is achieved when sufficient nutrients are consumed to support day-to-day body needs.
Undernutrition is
when nutritional reserves are depleted or nutrient intake is inadequate to meet day-to-day needs.
Overnutrition is
caused by consumption of nutrients, especially calories, sodium, and fat in excess of body needs.
The age groups at most risk for undernutrition are
infants, children, adolescents, and aging people.
Their nutrition is inadequate due to
not consuming enough food and nutrients in aging people. For children and infants they are solely reliant on their parents for their dietary choices so they may not be receiving the proper nutrition needed.
Purpose of Nutritional Assessment
Obtain a health history relevant to nutritional status. Inspect relevant systems for clinical signs and symptoms suggestive of nutritional deficiencies. A nutritional assessment is needed to determine if the patient is receiving adequate nutrition or if they are at risk, or suffering from malnutrition.
Types of Nutritional Assessment
24 hour recall. Food frequency questionnaire. Food diaries or records. Direct observation of feeding with documentation.
