DSM Criteria for Substance Use Disorder
2 of the following:
- Failure to meet obligations
- Repeated use when it is physically dangerous
- Repeated relationship problems
- Continued use despite problems
- Tolerance
- Withdrawal
- Substance taken in greater amounts than intended
- Efforts to reduce don’t work
- A lot of time spent trying to get substance
- Social activities reduced
- Craving is strong
Severity rating for substance use disorder
Mild: 2-3 criteria
Moderate: 4-5 criteria
Severe: 6 or more criteria
Tolerance
Larger doses needed to get same effect or effect of drug becomes markedly less when taking usual amount.
Substance withdrawal symptoms
- Muscle pains
- Twitching
- Sweats
- Vomiting
- Diarrhea
- Insomnia
Gambling Disorder Criteria
4 to meet diagnosis
- Gambling when feeling negative emotions
- Trying to stop causes negative feelings
- Reliance on other people to fix financial consequences
- Continued gambling despite problems
- Continued gambling to recoup losses
- Lying about gambling
- Need to gamble with more money
- Can’t stop gambling
- Time spent thinking about gambling
Delirium tremens (DT’S)
Person becomes delirious due to sudden drop in alcohol level in the blood
Fetal alcohol syndrome
Growth of the fetus is slowed.
Cranial, facial and limb abnormalities
Effects of secondhand smoke
- Lung damage
- Babies of women exposed to secondhand smoke more likely to be born prematurely and have birth defects
- Children of smokers more likely to have asthma and infections in ear
Effects of opioids
Euphoria, drowsiness and lack of coordination
Withdrawal symptoms of opioids
- Muscle pain,
- Sneezing
- Sweating
- Tearful and yawning
- Symptoms of flu
Etiology of substance use disorders (Genetic factors)
Relatives and children of problem drinkers are more likely to get alcohol use disorder
Twin studies revealed that identical twins have higher chance to get alcohol use disorder.
Etiology of substance use disorder (Neurobiological factors)
- Dopamine big factor in all substance use disorders (Incentive-desensitization theory)
- Valuing short term over long term
- Vulnerability model: problems in dopamine system make some more vulnerable
- Toxic effect model: problems in dopamine system are an effect of substance abuse
Etiology of substance use disorder (Psychological factors)
- Inability to regulate negative emotion
- Expectancy about effects of drug
- Personality factors (neuroticism, impulsivity, sensation seeking)
Incentive-sensitization theory
Dopamine system linked to pleasure becomes super sensitive to drug or cues related to drug
Etiology of substance use disorders (Sociocultural factors)
Countries where drinking is more accepted has higher dependency on alcohol
Exposure to alcohol in family increases likelihood of drinking
