Skilled Nursing Facility

A

Provides less intensive level of care than that found in a traditional/long-term acute care hospital; Patients may stay in SNF for a few days or as long as 100 days; Consists of skilled nursing care & physical, occupational, speech therapy as needed
SNF level of care usually is covered by Medicare & most private insurers, but there are certain requirements, such as:
- patient must have been hospitalized for at least 3 days before admission
- must enter the nursing home within 30 days of hospitalization
- there is a 100-day stay maximum per year related to any one hospitalization & diagnosis
- must be making regular progress as documented by the medical professionals

Rehabilitation Facility

A

is a level of care in which the patient can receive intensive physical, occupational, & speech therapy services
- patient must capable of participating in at least 3 hours of therapy a day to admitted to rehab facility
- focuses on treating patients with chemical dependency & mental health issues
- some facilities provide medical care in form of detoxification: removal of drugs & alcohol from person’s body (usually takes several days)

Long-term care facilities (nursing home)

facilities are where residents often live for many years; nursing care provided around the clock for residents in these facilities (chronic illnesses/disabilities)
