Assessment
The registered nurse collects comprehensive data pertinent to the patient’s health or the situation.
Diagnosis
The registered nurse analyzes the assessment data to determine the diagnosis or issues.
Outcomes identification
The registered nurse identifies expected outcomes for a plan individualized to the patient or the situation.
Planning
The registered nurse develops a plan that prescribes strategies and alternatives to attain expected outcomes.
Implementation
The registered nurse implements the identified plan .
Evaluation
The registered nurse evaluates progress toward attainment of outcomes.
Functional Nursing
is a model of care that is TASK focused, not patient focused. (places more emphasis on nurses’ following rules, regulations and policies’ and does not promote nurses’ decision making, autonomy, or professional development.)
Team Nursing
An RN leads a team composed of other Rn’s, LPN/LVNs and nursing assistive personnel or technicians.
Total patient care
original care delivery model developed during florence nightingale’s time -An RN is responsible for all aspects of care for one or more patients during an assigned shift.
Primary Nursing
the aim of placing RNs at the bedside and improving the professional relationships among staff members. (Nurse and patient relationships)- Same patients during the hospitalization visits.
Case management
requires an RN to maintain responsibility for patient care from admission to discharge.
Decentralized management
in which decision making is made at the staff level.
