What is LOC?
The person is awake, alert, and aware of stimuli from the environment and within the self
Orientation
The awareness of the objective world in relation to the self, including person, place, and time
Alert
Patient is fully awake and can respond to stimuli
Lethargic
Not fully alert
Drifts off to sleep when not stimulated
Can be aroused to name when called in normal voice but looks drowsy
Obtunded
Sleeps most of time
Difficult to arouse
Needs loud shout or vigorous shake
Stupor
Spontaneously unconscious
No response to pain or any external or internal stimuli
Delirium
Clouding of consciousness
Inattentive
Incoherent conversation
Aphasia
Defect in word choice and grammar or defect in comprehension
Defect is in higher integrative language processing
