Atelectacic crackles vs crackles
Crackles are heard when sections of alveoli are expanded by a few deep breaths and generally don’t disappear during inspiration
Atelectacic crackles are short, popping, crackling sounds that last only a few breaths
Barrel chest
Ribs are horizontal instead of downward slope
Normal with aging
Also associated with chronic emphysema and asthma
Bronchophony
Ask person to repeat ninety nine while you listen over the chest wall
Normal transmission is soft, muffled and indistinct
Fremitus
A palpable vibration from the spoken voice felt over the chest wall
Wheeze
High-pitched, musical, squeaking adventitious lung sound
Pleural friction rub
Very superficial sound that is coarse and low pitched
Sounds just like crackles, but close to the ear
Stridor
High pitched, monophonic inspiratory crowing sound
Egophony
Auscultate the chest while person phonates a long “ee-ee-ee-ee” sound
Hemoptysis
Coughing up blood from the lungs or bronchials
Kyphosis
An exaggerated posterior curvature of the thoracic spine that causes significant back pain and limited mobility
Orthopnea
Difficulty breathing when supine
Tactile fremitus
Palpating over the lung apices in the supraclavicular areas
Cheyne-stokes
A cycle in which respirations gradually wax and wane in a regular pattern
Whispered pectoriloquy
Ask the person to whisper a phrase such as one two three as you auscultate
The normal response is faint, muffled, and almost inaudible
