How many arteries and veins are in the umbilical cord and what is the pathophysiology?
The umbilical cord has 2 arteries and 1 vein. The arteries care deoxygenated blood away from the fetus heart to the placenta and the vein carries oxygenated blood back to the fetus heart.
First Trimester
Last menstrual cycle to 12 wks
Second Trimester
13 weeks to 27 weeks
Third Trimester
28 weeks to 40 weeks (Term)
What kind of changes happen in the mother as the fetus grows?
- Total Blood Volume increase by 40 - 50%
- RBC increase by 30%
- The HR elevates 10 - 20 bpm
- Cardiac Output rises by 30%
Preeclampsia
hypertension that occurs in pregnant women also referred to as pregnancy induced hypertension PIH. Preeclampsia is defined as an increase in BP after the 20th week of gestation. It is accompanied by protein release in the urine and often by edema
Eclampsia
is a seizure in a pregnant woman who has eclampsia and no other cause for the seizure
Supine Hypotensive Syndrome
hypotension when lying down or vena cava compression syndrome which may be an issue when a woman is injured and must be immobilized
What happens to the BP of a pregnant patient in the second trimester?
The systolic BP will drop 10 - 15 mm Hg and go back to baseline around the third Trimester.
When should you suspect preeclampsia in a pregnant patient?
You should suspect preeclampsia if the blood pressure of a woman in her second or third trimester of pregnancy is above 140 systolic or 90 diastolic.
How do you manage a prolapsed cord?
Your goal in the treatment of this patient is to prevent the woman from pushing and compressing the umbilical cord.
If possible place the Pt. in the Trendelenburg Position.
You must now insert a gloved hand into the vagina and gently push the presenting part away from the umbilical cord until you receive a pulse
