Why do we do an assessment, and why does it matter why we do an assessment?

- Gather baseline data
- Make subjective date robust
- Identify and confirm nursing diagnoses.
- Inform plans
- Evaluate the outcomes of care.

A
- Prepare space- is it clean, quiet, private?
- Is the equipment clean, working, appropriate?
- Is the patient positioned well, informed?
- Are you considering special factors- age, sociological issues, etc

How do you organize an assessment?
A
*Systematic and organized, head to toe, etc
* Start with the most important, concerning systems, End with painful procedures.
* During, look for symmetry, offer rests, take quick notes
