Ch 3-Thorax: Wall Flashcards
Superior Thoracic aperture borders:
- vertebra TI
- rib I
- manubrium of sternum
Inferior Thoracic aperture borders
- vertebra TXII
- rib XII
- rib XI
- costal margin
- xiphoid process
Articulation with ribs by:
- 2 demifacets,
- superior costal facet-head of own rib
- inferior costal facet-head of rib below - Transverse costal facet-turbercle of own rib
Special articulations of:
- TI
- TX
- TXI and TXII
- head of rib 1 wi/ superior costal facets.
- only with own rib X, no inferior costal facets.
- only with own ribs, lack transverse costal facet
- True ribs:
- False ribs:
- Floating ribs
- 1-7
- 8-12:
- 11 and 12
At Head of rib:
- superior costal facet with
- inferior costal facet with
- internal surface of anterior part of ribs faces somewhat superiorly relative to posterior part
- inferior costal facet above
- superior costal facet below
Rib 1
- slopes inferiorly to manubrium
- connects to TI by only one articular surface
- scalene tubercle distinct
- Anterior groove-subclavian vein
- Posterior groove-subclavian artery
Rib II
- like rib 1
- twice as long
Rib X
-has single articulation with its own vertebra
Ribs XI and XII
floating ribs!
- only with theire own vertebrae
- have no tubercles or necks
- short, little curve
- pointed anteriorly
Manubrium of sternum
- has jugular notch
- site for clavicle
- lateral sites for rib 1
Body of sternum
- superiorly theirs facet for rib 2
- lateral facets for ribs III to VI
xiphoid process
-articulation with 7th costal cartilage
Costotransverse joints
- synovial joints between tubercle of ribs and transverse process of related vertebra
- costotransverse ligament-attaches neck of rib to transverse process
- lateral costotransverse ligament-attaches tips of transverse process to nonarticular part of tubercle
- superior costotransverse ligament-attaches superior surface of neck to transverse process of vertebra above.
- Sternocostal joints
- rib 1
- rib II-VII
- @ rib II
- consists of fibrocartilaginous connection
- synovial joints
- at synovial joint, divided into 2 compartments by a intraarticular ligament
- Interchondral joints
- at Ribs V,VI to VII-X
- between costal cartilages of adjacent ribs
- synovial, reinforced by interchondral ligaments
- indirect anchorage to the sternum
Manubriosternal and xiphisternal joints
- joints between manubrium and body of sternum
- between body of sternum and xiphoid process
- called symphyses.
- slight angular movements occur between manubrium and body of sternum during respiration.
- forming sternal angle for palpitation of rib II
-sternal angle
- lies on horizontal plane that passes through intervertebral disc between TIV and TV
- separates superior mediastinum from inferior mediastinum
- marks superior border of pericardium
Muscles of pectoral region originate from anterior thoracic wall and insert into bones of upper limb
- Pectoralis major
- pectoralis minor
- subclavius muscle
Pectoralis major:
- origin
- insertion
- function
- medial half of clavicle, sternum, first 7 costal cartilage
- proximal humerus
- adduction, rotation, flexion of humerus
subclavius
- small, underneath pect major
- passes laterally from anterior medial rib I to inferior clavicle
- pulls tip of shoulder inferiorly
pectoralis minor
-clavipectoral fascia
- passes from ant surfaces of rib III to V to coracoid process of scapula
- pulls tip of shoulder inferiorly
- encloses subclavius and pec minor and attaches to clavicle above and floor of axilla below
- structures pass through all this
Cervical ribs:
-thoracic outlet syndrome
- rare, articulates w/ CVII, with the ant end attached to superior border of ant aspect of rib I
- structures that normally pass over rib I are elevated by and pass over, cervical rib and band
- abnormal compression of brachial plexus of nerves as it passes over the first rib and through axillary inlet into upper limb
