what type of cervical cancer is sometimes missed in cervical screening?
adenocarcinoma
who should get a cervical smear?
- all women between the ages of 25-64 years
- every 5 years
can women over 64 years get a cervical smear?
no
what are the expections to the cervical smear?
- delayed until 3 months post-partum
- never been sexually active can opt out
when is the best time to take a cervical smear?
mid-cycle
what is the cervical screening process?
- sample tested for high-risk strains of HPV
- cytological examination ONLY if hrHPV +ve
what is negative hrHPV?
no high-risk strains of HPV detected
what is test of cure pathway?
individuals who have been treated for CIN 1,2 or 3 should have a repeat screen 6 months later
what should a patient with a negative hrHPV do?
return to normal recall
unless:
* test of cure pathway
* untreated CIN1 pathway
* follow-up for incompletely excised cervical pathology
* follow-up for borderline changes in endocervical cells
what happens if sample is hrHPV +ve?
examined cytologically
what cytology results would warrent an abnormal result?
- borderling changes in squamous or endocervical cells
- low-grade dyskaryosis
- high-grade dyskaryosis (moderate or severe)
- invasive squamous cell carcinoma
- glandular neoplasia
what should a patient with a +ve hrHPV and an abnormal cytology do?
go for colposcopy
what should a patient with a +ve hrHPV but normal cytology do?
test is repeated at 12 months
hrHPV +ve + normal cytology + -ve hrPHV
return to normal recall
hrHPV +ve + normal cytology + +ve hrPHV but cytology still normal
further repeat test 12 months later
