Screening
Patient info is reviewed to determine appropriateness for CM.
Assessing
Various assessments to understand the situation and develop a plan of action.
Stratifying Risk
Classifies patient into a low, moderate or high category. Helps prioritize cases and determine the level of intervention needed.
Case Planning
Develop an individualized case management plan. Communicate available resources, and promote safety and quality of care to help patients meet their health needs.
Implementing
aka Care Coordination. Involves putting the patient’s treatment plan into action. Guides patient’s through treatment options they established in the planning phase.
Following Up
Check in with patients, usually after a crisis or a change in their service or treatment plan. Can help stabilize unstable situations.
Transitioning
Assessing whether patient is ready to transfer to another level of care, facility, provider or d/c home. Safety is key concern.
