Case 50: An outbreak of infections in an intensive care ward Flashcards
What is the Gram stain for pseudomonas aeruginosa?
Gram negative rods
What is the colonial morphology of Ps. aeruginosa on HBA?
No growin in AnO2
O2: green, slimy (beta haemolysis) - although it looks like alpha haemolysis because it’s green
Strict aerobe
What is the colonial morphology of Ps. aeruginosa on MAC?
Yellowish, slimy
What is the colonial morphology of Ps. aeruginosa on Cetrimide agar?
Light green, slimy
How does alpha haemolysis appear on HBA?
Greening
How does beta haemolysis appear on HBA?
Clearing
Why is HBA indication of alpha or beta haemolysis unreliable with ps. aeruginosa?
Because ps. aeruginosa is green anyway
Why is cetrimide agar used for identification of ps. aeruginosa?
Because it is a commonly used hospital disinfectant - ps. aeruginosa is very good at growing in a hostile environment
Which common antimicrobials is ps. aeruginosa resistant to in nosocomial infections?
All: piperacillin, ticarcillin and cefotaxime
Ticarcillin is ordinarily for pseudomonas
What is piperacillin?
Synthetic beta lactam designed to combat ps. aeruginosa
What is used in conjugation with ticarcillin to treat ps. aeruginosa?
Clavulanic acid
What is used in conjugation with piperacillin to treat ps. aeruginosa?
Tazobactam
Which antimicrobials is ps. aeruginosa susceptible to?
Piperacillin + tazobactam
Aztreonam
Ticarcillin + clavulanic acid (expensive)
What is tazobactam?
Beta lactamase inhibitor that targets ps. aeruginosa
What is the recommended empiric treatment for ps. aeruginosa?
Piperacillin and tazobactam
