Cardo Drugs Flashcards
What do diuretics do?
Block Na+ reabsorption in kidneys
What are the 2 types of diuretic?
Thiazide
Loop
Give an example of a thiazide diuretic and what it is used to treat
Bendrofluazide
Used in Hypertension
Give an example of a loop diuretic and what it is used to treat
Furosemide
Used in heart failure
What are the side effects of diuretics?
Hypoalkaemia (low K+)
Hyperglycaemia (diabetes)
Increased uric acid (Gout)
Impotence
Give an example of a cardioselective beta blocker and what it is used to treat
Atenolol
Angina, hypertension, heart failure
Give an example of a non-selective beta blocker and what it is used to treat
Propanolol
Thyrotoxicasis
What are the side effects of beta blockers?
Tiredness
Heart failure (good in medium/long term HF but can worsen short term HF)
Cold peripheries
Asthma (never use in established asthma)
What do calcium antagonists do?
Limit the increase in intracelluar calcium and hence limit the increase the contractility of cardiac and smooth muscle cells.
What are the 2 types of calcium antagonists?
Dihydropyridines
Rate limiting calcium antagonists
Give an example of a dihydropyridine and what it is used for
Amlodipine
Used in Hypertension and angina
What are the side effects of dihydropyridines?
Ankle oedema
Give an example of a rate limiting calcium antagonist and what they are used for
Verapamil, (diltiazem)
Used in hypertension, angina, AF and SVT
What should rate limiting calcium antagonists not be used with (except with extreme caution)
Beta blockers
What do alpha blockers do?
Block alpha adrenoceptors to cause vasodilatation
What are alpha blockers used in? and give an example of one.
Used in hypertension and prostatic hypertrophy
Doxazosin
What are the side effects of alpha blockers
Postural hypotension
What do ACEIs do?
Block angiotensin I from becoming angiotensin II
Give an example of an ACEI and what they are used for.
Lisinopril
Used in hypertension and heart failure
What are the side effects of ACEIs
Cough, renal dysfunction, angioneurotic oedema
What do ARBs do?
Block angiotensin II receptors
What are ARBs used in? and give an example of an ARB
Used in Hypertension and heart failure
Losartan
What are the side effects of ?
Renal dysfunction
What do nitrates do?
Relax all types of smooth muscle (clinical doses = venodilators)
What are nitrates used in and give an example of a nitrate
Used in angina and acute heart failure
Isosorbide mononitrate
What are the side effects of nitrates?
Headache and hypotension/collapse
What do antiplatelet agents do?
Prevent new thrombosis from forming
Give examples of antiplatelet agents + what they are used for.
Aspirin + Clopidogrel
Used in Angina, acute MI, CVA/TIA, patients at risk of MI and CVA
What are the side effects of antiplatelet agents?
Haemorrhage anywhere, peptic ulcer haemorrhage, aspirin sensitivity leading to asthma
What do anticoagulents do?
Prevent new thrombosis from forming
Give 2 examples of anticoagulants.
Heparin and Warfarin
What are anticoagulants used in?
DVT, PE, NSTEMI, AF
What are the side effects of anticoagulants?
Haemorrhage anywhere
What is the action of anticoagulants reversed by?
Vitamin K
What do fibrinolytic drugs do?
Dissolve already formed clots
Give examples of fibrinolytics.
Streptokinase
Tissue plasminogen activator (tPA)
What are fibrinolytics used in?
STEMIs, PE and CVA (selected cases)
What are the side effects of fibrinolytics?
Serious risk of haemorrhage
In what cases should fibrinolytic drugs be avoided?
Recent haemorrhage Trauma Bleeding tendancies Severe diabetic retinopathy Peptic ulcer
What do anticholesterol drugs do?
Block HMG CoA reductase
What are the 2 types of anticholesterol drugs?
Statins
Fibrates
Give an example of an statin and state what cases it would be used in.
Sinvastatin
Hypercholesterolaemia, diabetes, angina/MI, CVA/TIA, and those at risk of MI or CVA
What are the side effects of statins?
Myopathy and Rhabdomyolysis renal failure
Give an example of a fibrate and state what they are used for.
Bezafibrate
Used in hypertriglyceridaemia and low HDL cholesterol
When is adenosine used?
In the acute phase of supraventricular arrhythmias
What other drugs are used in arrhythmias?
Amiodarone (long acting)
Beta blockers
Flecainide (for tacchyarrhythmias)
What are the side effects of the anti-arrhythmic drugs?
Phototoxicity, pulmonary fibrosis, Thyroid abnormalities, thyroid abnormalities
What are the 2 effects of digoxin?
- Blocks AV conduction - thus increases AV conduction delay
2. Increased ventricular irritability which produces ventricular arrthymias
What is digoxin used for?
AF
What are the signs of digoxin toxicity?
N&V
Yellow vision
Bradycardia and heart block
Ventricular arrhythmias
