How does warfarin work as an antioagulant?
A
inhibits reduction of vit K to it’s active form hydroquinone which helps carboxylate (1972) of the CC and protein C

Target INR for VTE, AF and for mechanical heart valves?
A
VTE prophylaxis: 2.5 (if recurrent then 3.5)
AF: 2.5
Mechanical HV’s: dependent on valve (M higher)

Factors that may potentiate warfarin?
A
- p450 enzyme inhibitors: amiodarone, ciprofloxacine
- Liver disease
- Cranberry juice
- NSAIDs

what should you give for VT acutely
A
amiodarone

what do you see on ECG with hypothermia
A
Bradycardia (<60bpm) and not tachycardia J waves Prolonged PR, QT and QRS intervals Shivering artefacts VT, VF or asystole
