mechanism of ace inhibitor
block angiotensin converting enzyme = vasodilation
use of ace inhibitors
first line for hypertension in white and under 55. AND ALL DIABETICS
side effects of ace inhibitor
dry cough, renal dysfunction. dont use in pregnancy
ARB
losartan, valsartan, candesartan (sartan)
mechanism of ARB
angiotensin II receptor blocked = vasodilation
use for ARB
hypertension and heart failure. doesnt cause dry cough
side effects of ARb
oedema. dont use in pregnancy
calcium channel blocker
amlodipine (non-rate limiting) verapamil (rate limiting)
mechanism of CCB
block entry of Ca2 into cell = arteriolar smooth muscle relaxation. negative inotropic effects
uses of CCB
hypertension (not RL) angina ( RL) supraventricular arrhythmias (RL)
side effects of CCB
dizziness, hypotension. dont use in heart failure
thiazide diuretics
bendroflumethiazide
uses of thiazide diuretics. side effects
hypertension can cause hypokalemia
loop diuretics
furosemide
uses of loop diuretics, side effects
heart failure can cause hypokalemia
potassium sparing diuretics
spironolactone
uses of potassium sparing diuretics
heart failure, ascites/oedema. can cause hyperkalemia
beta blockers
propanolol, atenolol (olol)
mechanism of beta blockers
blocks B1 receptors from increasing rate, force and AV node speed = relaxation of muscle
uses of beta blockers
arrhythmias, angina, heart failure, hypertension (can be used in pregnancy)
side effects of beta blockers
cold peripheries, fatigue. dont use in asthma
muscarinic antagonists
atropine. increases heart rate. used in severe bradycardia. can cause dry mouth and blurred vision
nitrates
GTN spray. relaxes smooth muscle. used in angina. can cause hypotension, fainting and headache
