Campylobacter, Helicobacter, and Listeria Flashcards
Campylbacter are what shape? Gram _____, ____aerobic. They are found ________. Their reservoir is ________.
Campylobacter are curved Gram(-) rods microaerophilic, ubiquitous, reservoir farm animals
How does campylobacter present?
bloody diarrhea, may have some autoimmune complications
What are some possible complications of campylobacter infection?
Complications follow bacteremia: Guillain-Barre, reactive arthritis, HUS, meningitis, vascular infection.
How do you diagnose campylobacter?
Diagnose by microaerophilic culture, gram stain of fecal smear, sigmoidoscopy.
How do you treat campylobacter?
Treat with rehydration, add antibiotics if high fever or predisposed to complications (immunosuppressed, pediatric, worsening)
What is the pathogenesis of campylobacter?
Colonize both intestines, destructive intracellular infection, bloody diarrhea.
Pathogenesis of H. pylori
colonizes stomach, expresses urease, irritates stomach lining, attracts massive inflammatory infiltrate, destroys local cells → peptic ulcer. Predisposes to gastric cancer and MALT lymphoma. Infection much more common than symptoms.
How do you diagnose H. pylori? How do you treat?
Diagnose by scope, urea breath and/or PCR, biopsy, treat w/ approved trios of antibiotics
Listeria monocytogenes is gram _____, _____ shaped, _____-hemolytic, grows in _____ temps.
Listeria is a Gram(+) rod, beta-hemolytic, grows well in cold
How is listeria transmitted?
ingestion of uncooked/undercooked, particularly refrigerated, foods or contact w/ farm animals
Listeria pathogenesis
Listeria is intracellular; escapes from phagocytic vacuole using Listerolysin and transmits cell-cell by actin-based motility (ActA); will grow in placenta
Who does listeria primarily infect?
fetus (crosses placenta)–>morbidity and mortality
newborn (during delivery)–> morbidity and mortality
pregnant women –> complications of preg.
immunocompromised adults–> CNS infection
How do you treat listeria infection?
Treat with ampicillin+gentamicin or trimethoprim+sulfamethoxazole.
how does listeria present in previously-healthy people?
uncomplicated gastroenteritis ; tx: rehydrate and report
