C5- PLAX Colour And Spectral Flashcards
What effect does a broken chordae have of blood flow?
Causes regurgitation
What’s the most obvious sign for regurgitation?
Aliasing and flow reversal when the valve is closed
How wide should you colour box always be?
Slightly wider than the annulus
Do we use CW or PW when measuring the peak velocity of he TV?
CW because flow is so fast
What is RVSP and what’s the formula?
RVSP= R ventricle systolic pressure
RVSP= 4 (peak V of TV regurg.)^2 +RAP
RAP is right atrial press.
When will the RVSP = SPAP
If there’s no obstruction in the RVOT
Norm CW peak V for TV? What press value does this translate to?
< 2.7 m/s…. 29mmHg
What 2 things are needed to assess RVSP?
Peak TV velocity
IVC sniff test
How is RAP (R atrial pressure) assessed?
How much should this structure collapse?
Sniff test in the subcostal 4 chamber view
Collapse > 50%
What happened to IVC when the RA pressure is high
It will be dilated and wont collapse with sniffing
Where do we place calipers to measure IVC?
1-2cm back from Hepatic vein
If the IVC is < or = to 21mm and IVC collapsibility is >50%, what value do we add for the RAP?
What does this say about RAP pressure?
3
It’s normal
If the IVC is < or = to 21mm and IVC collapsibility is <50%, what value do we add for the RAP?
What does this say about RAP pressure?
8
It’s moderate
If the IVC is > 21mm and IVC collapsibility is >50%, what value do we add for the RAP?
What does this say about RAP pressure?
8
It’s moderate
If the IVC is > 21mm and IVC collapsibility is <50%, what value do we add for the RAP?
What does this say about RAP pressure?
15
It’s high
