How many hours of continuing education are required and how many years do you have to complete them?
A person renewing his or her license or approval shall submit proof satisfactory to the board, that during the preceding two years, has completed the required number of CEU’s established by the board. Required continuing ed shall not exceed 30 hours every 2 years.
If you do not complete CEU’s, when does your license expire?
Any license or approval for the practice pf physical therapy shall expire at midnight on the last day of the birth month of the licensee or holder of the approval during the second year of a two year term, it not renewed.
How do you renew a license?
To renew an unexpired license or approval, the licensee or holder of the approval shall, on or before the dates on which it would expire, apply for renewal on a form prescribed by the board, pay the renewal fee, an submit proof of completion of units. They shall also disclose any misdemeanor or other criminal defenses for which he or she has been found guilty or to which they have pleased guilty or no contest.
How many years do you have to renew your license once it has expired?
A license that has expired may be renewed within 5 years upon payment of all accrued and unpaid renewal fees and satisfaction of the requirements described.
When does the board notify you of your renewal fees?
**At least 60 days before the expiration of any license or approval, the board shall mail to each licensee, at the latest address furnished by the licensee to the executive officer, a notice stating the amount of the renewal fee and the date on which it is due, and that failure to pay it on or before the due date will result in expiration of the license.
