What are features of an infection of the breast?
Redness
Warm
Tender
Lumpy
What are microcalcifications?
Tiny deposits of calcium
Small % may be precancerous or cancer
What are the 7 factors in a breast profile?
Ducts Lobules Dilated section of duct holding milk Nipple Fat Pec. major muscle Chest wall/rib cage
What is the screening for breast cancer?
Mammogram
Age 50-70
every 3 years
What is the Triple Assessment of the breast?
Clinical examination
Breast imaging (mammogram, USS or both)
Fine needle aspiration or needle core biopsy if lump is found
When would you give Irradiation of the breast alone?
-ve sentinel node biopsy
Nodes containing micromets (>0.2mm)
<4 nodes involved in adequate axillary node clearance
When would you give boost to tumour bed?
Women under age of 54 on day of surgery
Posterior margin <1mm for invasive disease after full thickness excision
When would you give chest wall radiotherapy?
Tumour size >5cm
4 or more involved nodes
Involved resection margins
T4 disease
When would radiotherapy to the ipsilateral axilla be given?
> 1 +ve macromet sentinel node
Extensive ECS post axillary clearance
When would you offer radiotherapy to the Supraclavicular fossa?
4 or more involved nodes
What can be given alongside radiotherapy? (hormones)
Tamoxifen (10 years)
Letrazole (5 years)
In what way can chemotherapy be given?
Neoadjuvantly
To whom can trastzumab be given?
Patients whose cancer cells overexpress Her-2-neu as measured by IHC or FISH
