What 2 medications treat BPH?
- 5-Apha-Reductase Inhibitors
- Alpha1-Adrenergic Antagonists
What are the 2, 5-Apha-Reductase Inhibitors? What do they do? Adverse effects?
- Finasteride (Proscar)
- Ductasteride (Avodart)
- These decrease the size of the prostate by blocking hormones (decrease DHT)
- Adverse effects: make boobs, decrease libido and ejaculate
- prostate is 5x’s the size it should be, takes 5 months to kick in, makes your baby have 5 heads (teratogen)
What are the 2, alpha 1-adrenergic antagonists? What do they do? What are the adverse effects? What is an important nursing consideration for both of these medications?
- Tamulosin (Flowmax): ejaculation issues
- Terazosin (Hytrin): hypotension, fainting
- relax the smooth muscle to facilitate urination
- counsel pt about cataract surgery if they are on this medication
What is a non pharmacological consideration for BPH?
-self-cath
What are the minimally invasive therapies for BPH?
- TUMT: transurethral microwave thermotherapy
- Transurethral needle ablation (TUNA)
- Laser prostatectomy
What should a PSA score be?
less than 4.0
What findings from a DRE indicate BPH?
prostate feels firm, smooth, and symmetrically enlarged
Will BPH meds effect sexual relationships?
it should not effect an erection
Who needs to be counseled if they have BPH and are getting cataract surgery?
pt’s on alpha 1 adrenergic agonists
