Production function

Primary health care
Initial point of entry into a healthcare system. Includes primary care clinical services, health promotion, disease prevention, and rehabilitation.
Primary care
Initial point of entry into a healthcare system. Focuses on the provision of services to diagnose, treat, and manage disease.
Who usually performs primary care services?
General practitioners.
Patient rostering
A model of primary care that requires patients to formally register with a provider.
Virtually roastered
When a payer looks at billing records to see where a patient usually goes to receive care from the physician they usually see.
Ambulatory care
Services given outside of hospitals.
Urgent care
Problems that are less severe but cannot wait for an appointment with a family doctor.
Long term care
A set of health, personal care, and social services which are required on a sustained basis.
Three categories of long-term care
Acute care substitution, long-term care substitution, and prevention/maintenance.
Acute care substitution (LTC)
Allows people to be discharged from acute hospitals to the community and still receive the services they need.
Long-term care substitution
Serves those who might otherwise have to be in a LTC institution.
Prevention/maintenance (LTC)
Keeps people healthy enough to remain out of institutions.
Social supports
Help with things like doing the shopping, the laundry, and cleaning.
Activities of daily living
Involve the sorts of tasks that people normally do but may need help with, including things like functional mobility and personal care.
