What are the causes?

A

Dislodgment of needle, bleeding at access site, disconnection of lines, system clotted-unable to return blood

What are the symptoms?

A

Blood on floor, chair or clothing blood leak detector alarm, venous pressure alarm if needle dislodgement or line separation, circuit clotting, TMP alarm, visible clots in drip chamber

What is the intervention?

A

Observation of patient, maintain visible access, monitor machine pressure- reviewing trends (changes in TMP, venous pressure) proper anticoagulation, proper post tx access care (avoiding break through bleeding)

What is the prevention?

A

Re-educate patient on importance of leaving access uncovered
For continued non-adherence, involve IDT intervention
